I am over the age of 18 and do hereby make my application to become a member of the Palm Beach Soccer Club Ltd and request you enter my name on the Register of Members. If accepted, I agree to be bound by the provisions of the Memorandum & Articles of Association and any rules, regulations or by-laws of the Club from time to time in force. I also agree to abide by the decision of the Board of Management with regard to this application
